Welcome to Premier Gastroenterology’s Open Access Colonoscopy Program.
This program has been developed to meet an increasing demand for colonoscopies performed exclusively for screening for colorectal cancer. The American Cancer Society estimates more than 135,000 colorectal cancer cases in the United States for 2017. It is expected to cause about 50,000 deaths during the year.

Our Open Access Colonoscopy Program was developed for generally healthy patients who either decided to be screened on their own, or who were advised to be screened by their doctors. You are not required to be seen in our office prior to colonoscopic examination. Therefore, it is important for you to be familiar with details of the colonoscopic procedure and follow Steps 1–4 very carefully. It is also important to provide us with all pertinent and accurate health information before the decision for the procedure is made.
All forms can be downloaded from the link below, and submitted electronically for faster service. For additional information, as well as to request the necessary documents and forms in the mail, please call 281.392.0425, 9 am – 5 pm Monday through Thursday, 9 am – 3 pm Friday.
It is important that all steps below are followed carefully, including reading the necessary files referenced on each step. It is up to you as the patient to read and follow the instructions carefully. To download the form for completion, you may need to download Adobe Reader from this link to the Adobe Web site.
Step 1
Please review general information about screening for colorectal cancer provided by the Center for Disease Control and entitled “Colon Cancer Screening.” Please also carefully read the document entitled “Colonoscopy.” It will provide you with information about the colonoscopic technique, its applications, and possible risks and complications of the procedure.
Step 2
Please print and complete the “Pre-colonoscopy Patient Questionnaire.” After completion, please email the form to us at contact@pgkaty.com. If you cannot email, you can fax it to us at 281.392.0250. You can also mail it to us or drop it by the office in person.
Once we receive the questionnaire via email, fax, mail or in person, you will be contacted by one of our employees within 3–5 business days. They will complete the pre-screening process and answer any questions you may have at that time. Please remember that you will be examined by the physician performing your colonoscopy prior to the procedure. At that time you will have the opportunity discuss any remaining issues.
Appointment times vary, and can be booked within a few days to a few weeks from the time of the request, depending on volume, so please send in the information as soon as possible so we can accommodate your schedule.
Step 3
Your colonoscopy will be performed by Dr. Kevin Marks, an experienced gastroenterologist who has successfully performed thousands of colonoscopies.
